Job summary

Adventist Health is seeking a Home Health Physical Therapist in Fort Bragg, California. The role offers an hourly wage with a $10,000 incentive and relocation assistance. As staff, you will join an interdisciplinary team to restore function and independence for patients in home care settings.

The position requires a PT license in the state, CPR/BLS, and a valid driver's license. A Bachelor's degree is required, Master's preferred.

Qualifications

• Bachelor’s Degree or equivalent (some certifications may require Bachelor’s).
• Master's Degree preferred.
• Physical Therapy licensure in state of practice required.
• CPR/BLS certification required.
• Valid Driver’s License and 21+ age required.

Responsibilities

• Plans and conducts segments of treatment programs to restore and improve functions.
• Co-leads interdisciplinary patient groups and coordinates care.
• Performs comprehensive clinical documentation within deadlines.
• Supervises assigned assistants and ensures discharge plans are communicated.
• Performs other related duties as assigned.
