Position Summary

As a trusted leader in home health and hospice care, we’re seeking a skilled Physical Therapist who is driven by compassion, clinical excellence, and a desire to help patients regain independence in their own environment.

About The Role

As a Physical Therapist, you’ll deliver personalized care to patients with a wide range of diagnoses and disabilities—all within their homes. Guided by a physician’s plan of care, you will assess, treat, and consult to help patients reach their highest level of functional independence. Your work will reflect sensitivity to each individual’s age and unique needs, ensuring care is always delivered with respect and empathy.

Patient Population Served

Adolescent, Adult, Geriatric. The employee must be competent in interpreting a patient's self‑report or behavior, with an understanding of cognitive, physical, emotional/psychosocial, and chronological maturation processes. Treatment should be individualized, reflecting developmental needs and range of treatment options for each patient.

Position Specific Responsibilities
  • Plan and administer medically prescribed physical therapy treatment programs for patients to restore function, relieve pain, and prevent disability following disease, injury, or loss of body part.
  • Complete comprehensive physical assessments and identify a need for referral to additional appropriate clinical disciplines.
  • Perform patient tests, measurements, and evaluations such as range‑of‑motion and manual muscle tests, functional analysis, and body‑part measurements.
  • Instruct patient and family in physical therapy procedures to be continued at home.
  • Supervise licensed physical therapy assistants and aides.
Education and/or Experience
  • Required – Bachelor’s degree in Physical Therapy from an institution accredited by the Commission on Accreditation in Physical Therapy Education, or
  • Master’s degree in Physical Therapy from an institution accredited by the Commission on Accreditation in Physical Therapy Education, or
  • Doctorate degree in Physical Therapy from an institution accredited by the Commission on Accreditation in Physical Therapy Education.

Preferred – One year of Physical Therapy experience with adults in a home health care or rehabilitation setting.

Certifications, Licenses, and Registrations
  • Required – Current, unencumbered (State Employed) License as a Physical Therapist.
  • Required within 30 days of hire – National Provider BLS – American Heart Association (Vendor Managed).
Physical Demands And Work Environment

Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. Expectations include exposure to bodily fluids, infectious diseases, and typical healthcare conditions. Routine use of standard medical equipment and tools associated with clinical care is essential.

Compensation Range

$50.56–$73.29 (base pay). Additional compensation may be available, such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
