CenterWell Home Health is seeking a licensed Physical Therapist to join our patient-centered team in Virginia Beach. You will provide in-home PT to help patients regain strength, mobility and independence, coordinating with physicians and care teams to meet individualized goals.

This full-time role requires a current PT license and CPR certification, with responsibilities including assessment, treatment planning, documentation and guidance to PTAs and Home Health Aides.

Responsabilidades

• Plan and administer prescribed physical therapy in patients' homes to improve function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program aligned with physician's Plan of Treatment and patient goals.
• Provide PTAs and Home Health Aides with written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family; submit evaluations, plans, progress notes, and discharge summaries.

As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility and independence, from the comfort of their homes. By delivering personalized care that focuses on functional improvement, you'll empower individuals to overcome limitations and enjoy a better quality of life.

As a Home Health Physical Therapist, you will:

• Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.

Required Qualifications

• Current and unrestricted Physical Therapy license
• Current CPR certification
• Experience collaborating with a team of healthcare professionals

Preferred Qualifications

• One year of experience as a Physical Therapist

Pay Per Visit/Unit Rate

$59.00 - $83.00

Additional Information

TB Statement: This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driving Statement: This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $93,000 - $128,000 per year.
