Health Here in Pensacola, FL is seeking a Physical Therapist to provide in-home, patient-centered care. You will assess functional status, develop individualized treatment plans, and deliver evidence-based interventions to improve mobility and safety in the home environment.

You will collaborate with nurses and other healthcare professionals, educate patients and caregivers, and ensure accurate documentation of care delivered at the patient’s residence.

Formación

• Licensed Physical Therapist with current license to practice in Florida.
• Strong communication and collaboration with patients, families, and teams.
• Ability to design personalized treatment plans based on patient needs.

Responsabilidades

• Assess patients' functional status and design individualized home-based treatment plans.
• Deliver evidence-based interventions to improve mobility, safety, and independence at home.
• Collaborate with nurses and healthcare professionals in an interdisciplinary team.
