Job summary

CARE ESSENTIALS HOME HEALTH is seeking a licensed Physical Therapist to service Perris, Hemet, Murrieta, Lake Elsinore, Menifee and surrounding areas. You will assess patients, design personalized plans, and implement therapeutic interventions in home settings to improve daily living skills.

You will monitor progress, adjust plans as needed, and educate patients and families while maintaining thorough documentation.

Qualifications

• Current state licensure to practice as a Physical Therapist.
• Strong communication and interpersonal skills.
• Experience in home health care preferred.
• Ability to work independently and as part of a team.
• Compassionate demeanor with a patient-centered approach.
• Solid organizational skills and attention to detail.
• Willingness to travel to patients' homes as needed.
