Job summary

Angelcare Home Health Services is seeking a Physical Therapist to provide clinical PT services under a physician's plan of care in Milpitas, CA. The role includes program initiation, patient and caregiver training, and contributing to discharge planning.

Qualified candidates hold a CA PT license, have at least two years of PT experience (home health preferred), and maintain CPR certification. Strong communication and organizational skills are essential for success in this role.

Qualifications

• Requires a degree in physical therapy approved by an accredited organization.
• Licensed to practice as a physical therapist in California.
• Two years of relevant work experience, preferably in community/home health.
• CPR certification must be current.
• Excellent verbal and written communication and organizational skills.

Responsibilities

• Provides physical therapy services to patients according to a physician's plan of care and state practice act.
• Initiates PT program and trains other personnel and family/caregiver members on PT goals.
• May train patient in the use of a prosthetic device.
• Participates in discharge planning for patient.
• Prepares and submits clinical and progress summaries based on goals attained.
• When therapy is the only skilled service, instructs and supervises home health aide care every two weeks.

Overview

The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

• Provides physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act.
• Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
• May train patient in the use of prosthetic device.
• Participates in discharge planning for patient.
• Prepares and submits clinical and progress summaries based on the attainment of goals.
• When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two (2) weeks.

Qualifications

• Possesses a degree from a baccalaureate or master's program in physical therapy approved by an accredited organization.
• Licensed to practice as physical therapist by the Physical Therapy Board of California.
• Two years of appropriate experience as a physical therapist. Community/home health experience is preferred.
• Possesses and maintains current CPR certification.
• Demonstrates good verbal and written communication and organization skills.
