Job summary

Optimal Care is seeking a Home Health Physical Therapist to provide one-on-one rehabilitation in patients' homes within the Jackson, MI area. You will assess functional abilities, develop individualized treatment plans, and guide interventions to improve mobility, balance, strength, and safety.

In this role you will work with an interdisciplinary team, educate patients and caregivers, and travel daily to patient homes.

Qualifications

• Master’s or doctoral degree in Physical Therapy.
• Current state license as a Physical Therapist.
• Minimum 1 year of experience as a licensed Physical Therapist.
• Willingness to travel daily for home visits within service area.
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage.
• Preferred home health experience.
• OASIS certification or willingness to obtain.

Responsibilities

• Provide one-on-one rehabilitation in patients’ homes.

About the Role

Help patients regain strength, restore mobility, and return to the activities that matter most in their daily lives. As a Home Health Physical Therapist, you‘ll provide one‑on‑one rehabilitation care in patients’ homes, helping them recover from surgery, injury, or illness while improving function, reducing pain, and supporting greater independence.

This role is ideal for a licensed Physical Therapist who values autonomy, clinical variety, and the ability to build meaningful relationships with patients over the course of their recovery. You‘ll assess each patient’s functional abilities and home environment, develop individualized treatment plans, and guide them through targeted interventions that improve mobility, balance, strength, and safety. Working closely with an interdisciplinary care team, you‘ll educate and empower patients and caregivers while helping individuals make measurable progress toward their goals and a higher quality of life.

Location

• Office Location: 801 Rosehill Road, Jackson, MI 49202
• Main Service Area:Jackson, MI
  • This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes

Hours

• Monday to Friday, 8 a.m. to 5 p.m.

Required Qualification

• Master‘s or doctoral degree from an accredited Physical Therapy program
• Current state license as a Physical Therapist
• Minimum 1 year of experience as a licensed Physical Therapist
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage
• Willingness to travel daily for home visits within service area

Preferred Qualifications

• Home health experience
• OASIS certification or willingness to obtain
• Experience with Medicare home health documentation

Pay Range

$93,000—$116,000 USD
