Job summary

CaresLink is seeking a full-time Physical Therapist to provide services in Lake County, Leesburg, The Villages, and surrounding areas according to a physician's plan of care.

You will assess and direct physical therapy, instruct patients and families, report patient responses to treatment, and participate in discharge planning while coordinating with home health teams. Florida PT license, Driver's license, CPR, and AHCA background screening are required.

Qualifications

• Active Florida Physical Therapist license.
• Valid Florida driver's license.
• Current CPR certification.
• AHCA background screening on file.
• Chest X-ray report or 2 Step TST or lab test (Quantiferon Gold).
• Doctor's Certificate of Good Health.

Responsibilities

• Assess and evaluate therapeutic/rehabilitative status.
• Direct physical therapy treatment per plan of care.
• Instruct patients and families in devices and exercises.
• Report changes to physician and document progress.
• Develop and adjust the plan of care with the team.
• Initiate therapy programs and educate staff and family.
• Coordinate with home health for services as needed.
• Prepare clinical/goals progress summaries.
• Participate in discharge planning.
• Provide PT consultation to home families/caregivers.
• Supervise PT assistants per policy and state regs.
• Instruct and supervise home health aides as needed every two weeks.

## About the Job-This is for the Fulltime position.-This position will cover Lake county, Leesburg, The Villages, and surrounding areas1. Provides physical therapy services to patients according to a written physician's plan of care. This may include, but will not be limited to:A. Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.B. Assesses for muscle strength, mobility, gait, ROM—potential for rehab.C. Directing physical therapy treatment.D. Instructing patients and families/caregivers in the use and care of therapeutic appliances.E. Determining priority needs for physical therapy.F. Reporting to the physician the patient’s reaction to treatment or changes in condition.2. Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient byparticipating in case conferences.3. May train patient in the use of prosthetic device.4. Identifies patient and family/caregiver needs for other home health services and refers as necessary.5. Prepares and submits clinical and progress summaries based on the attainment of goals.6. Participate in discharge planning for the patient.7. Prepares and submits a clinical progress summary based on the attainment of goals as directed by the Organization's policy.8. Provides physical therapy consultation to home families/caregivers when indicated.9. Provides in-service education programs for nursing organization personnel as needed.10. Participates in peer consultation process.11. Supervises physical therapy assistants according to organization policy and stateregulations.12.
