CenterWell Home Health in Everett, WA is seeking a Home Health Physical Therapist to deliver one-on-one care in patients' homes, helping them regain strength, mobility, and independence. You will work under the Clinical Manager in a patient-centered, goal-oriented program.

Responsibilities include planning and adjusting rehab plans, documenting progress, mentoring aides, and coordinating with physicians. A $10,000 sign-on bonus and $5,000 annual tuition reimbursement support professional growth;

Qualifications

• Current, unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with an interdisciplinary healthcare team.

Responsibilities

• Plan and administer prescribed physical therapy treatments in patients' homes to help them achieve their highest level of physical function
• Assess and document patients' physical strengths, limitations, and progress to support physician evaluation and treatment planning
• Develop and implement individualized rehabilitation programs consistent with the physician's plan of care and patient goals
• Provide clear care instructions to Physical Therapist Assistants and Home Health Aides and oversee their performance and documentation
• Accurately document patient assessments, interventions, and outcomes
• Communicate patient progress to physicians, the Clinical Manager, interdisciplinary care team, patients, and families
• Complete and submit all required evaluations, treatment plans, progress notes, and discharge summaries in a timely manner

TB Screening Requirement

This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program. Candidates selected for this position will be required to complete TB screening.

Driving Requirement

This role is part of Humana's driver safety program and requires a valid state driver's license and personal vehicle liability insurance. Associates must maintain insurance coverage that meets state minimum requirements or Humana's minimum requirements, whichever is greater.
