Job summary

CenterWell Home Health is seeking a licensed Physical Therapist to deliver in-home therapy, assessing patients and guiding rehab plans to restore function and independence. You will work under the Clinical Manager, collaborating with a multi-disciplinary team to ensure comprehensive care in patients' homes.

The role requires a PT degree, current license, CPR certification, and prior team-based healthcare experience. A driver’s license and TB screening are part of Humana’s program.

Qualifications

• Degree from an accredited Physical Therapy Program (APTA approved).
• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• One year of experience as a Physical Therapist.

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.

Pay Range

• $59.00 - $83.00 pay per visit/unit
• $93,000 - $128,000 per year pay

Additional Information

TB Statement: This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driving Statement: This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.

Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $93,000 - $128,000 per year
