Job summary

Granite Peak Home Health is seeking an Occupational Therapy Assistant who is a graduate and meets the requirements for certification from an AOTA-accredited program, maintains NBCOT certification, and is currently certified in the states where practicing. The role requires active licensure or regulatory status where applicable; two years of experience are preferred.

Qualifications

• Graduate and meet certification requirements for Occupational Therapy Assistant.
• NBCOT certification must be kept current.
• Currently certified in the state(s) of practice.
• Licensed or regulated as OT by the state, if licensure applies.
• Two years of experience preferred.

Responsibilities

• Understands and adheres to agency policies and procedures.
• Improves residual physical disabilities and returns patients to optimum function.
• Collaborates with home health staff in care planning.
• Performs skilled OT procedures as ordered and per care plan.
• Instructs patients and families in home programs and exercises.
• Participates in in-service education and QAPI activities.
• Prepares and updates patient records and care plans.
