Job summary

PruittHealth @ Home in Georgia is seeking a licensed therapy professional to evaluate, treat, and coordinate care for home health patients in the patient’s residence. You will apply evidence-based interventions, document progress, and contribute to the care plan with physicians and the interdisciplinary team.

The role requires an unrestricted state license, graduation from an ACOTE-accredited program, and the ability to work independently in a home setting.

Qualifications

• Must have a valid and unrestricted professional license in state of practice.
• Graduate of ACOTE accredited school.
• If foreign-trained, the program must be deemed substantially equivalent to a U.S. Occupational Therapy entry level education by a credentials evaluation organization approved by the AOTA.

Responsibilities

• Assesses, monitors, documents and reports progress of patient's health and condition using required documents via clinician preferred technology, and within established timelines
• Helps maintain continuity of care by collaborating with the interdisciplinary team and attending physician to develop, integrate, and manage the patient care plan of care
• Demonstrates intermediate knowledge of home care and competency in discipline for specific patient care skills
• Recognizes and responds to priorities in patient care with initiative and sound judgment
• Completes documentation timely, accurately, and at the point of care, according to industry standards
• Maintains current license, certification, and clinical records to meet federal, state and agency regulations and guidelines
