Job summary

Henry Ford Health in Detroit is seeking a Home Health Occupational Therapist to deliver in-home evaluations and individualized treatment plans, helping patients regain independence.

You will assess functional abilities, implement therapies, educate families on home safety, monitor progress, and collaborate with an interdisciplinary team to ensure holistic, high-quality care for patients in mid‑Michigan from our Jackson hospital campus.

Qualifications

• Graduate of an accredited occupational therapist program.
• Michigan state licensure as an Occupational Therapist.
• Valid Michigan driver’s license and dependable vehicle insured as required by the State of Michigan.

Responsibilities

• Assess patients’ functional abilities and develop personalized treatment plans.
• Implement therapeutic interventions to improve patients’ independence in daily activities.
• Educate patients and their families on adaptive techniques and home safety.
• Monitor progress and revise treatment plans as necessary to achieve optimal outcomes.
• Collaborate with interdisciplinary teams to ensure holistic patient care.
