Job summary

AdventHealth is seeking an experienced Occupational Therapist to provide comprehensive home health services in the North or South Lake County area in Florida. You will assess, plan, and implement therapy programs, collaborating with families and physicians to ensure optimal patient outcomes.

The role requires OT licensure, at least 1 year of relevant experience, and offers a sign-on bonus up to $5,000, mileage reimbursement, and a company-provided phone and laptop/iPad.

Qualifications

• Master's degree in Occupational Therapy preferred.
• Must possess OT license.
• 1+ years of OT clinical experience in home health preferred.

Responsibilities

• Coordinate home-based occupational therapy for patients and families.
• Perform functional evaluations and document results in medical records.
• Develop and implement patient-specific care plans with measurable goals.
• Educate patients and caregivers on therapy techniques and home programs.
• Maintain documentation and participate in interdisciplinary team conferences.
