Optum, in conjunction with Caretenders Home Health, seeks an Occupational Therapist to assess and evaluate patient care needs related to functional status, ADLs, and home safety within a home health setting.

Primary responsibilities include performing OT evaluations, coordinating with physicians on the care plan, and documenting CMS-relevant data while serving a diverse patient population.

Formación

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsabilidades

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy.
• Evaluates patient functional status and CMS documentation needs; consults with physician on plan of care.
• Observes, records, and reports patient response to treatment and changes to condition.
• Conducts patient assessments evaluating function using standard tests.
• Ability to work in any home situation regardless of patient background.
