Job summary

Health Link Home Health & Hospice is seeking an Occupational Therapist (OT) to cover San Francisco or San Mateo County. The role pays $60–$75/hour with a $10,000 sign-on bonus, and offers a steady full- or part-time schedule with an emphasis on efficient, AI-assisted charting.

You will evaluate and treat patients in their homes, develop individualized plans of care, educate families, and coordinate with a clinical team. Travel within your territory is required.

Qualifications

• Current California OT license is required.
• California driver's license required.
• Home health experience preferred; orientation provided.
• Ability to travel regularly within assigned territory.

Responsibilities

• Complete evaluations, re-evaluations, and discharges, and develop individualized plans of care based on physician orders.
• Provide skilled treatment focused on ADLs, adaptive equipment, home safety, and functional independence
• Educate patients and caregivers on safe, independent routines at home
• Coordinate with physicians, case managers, and the interdisciplinary team
• Complete documentation using AI-assisted tools
