Job summary

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for home health visits in the Oakton/Fairfax, VA area. Roles include per diem, part-time, or full-time opportunities serving adult and geriatric patients in senior living communities.

Responsibilities include conducting functional assessments, developing individualized plans, ensuring safe home environments, and educating patients and families. VA license and at least one year of OT experience are required.

Qualifications

• VA OT license is required.
• Minimum of one year of recent OT experience.
• Graduation from an OT program accredited by the AMA/AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills, organizational skills, and problem solving.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform evaluations of patients’ functional status including ADLs and IADLs.
• Assess cognitive, psychological and motor performance and develop intervention plans.
• Evaluate home environments for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Document observations and interventions using EMR.
• Educate patients and families on rehabilitative care and activities.
