Job summary

UnitedHealth Group is seeking an Occupational Therapist in Home Health to assess and evaluate patient care needs related to functional status, daily living activities, fine motor coordination, home assessments and adaptive equipment.

You will evaluate patient status, consult with physicians, and contribute to care plans while ensuring CMS documentation and compliance. A WA license and a valid driver's license are required; CPR certification is preferred or obtainable within 90 days.

Qualifications

• Current Occupational Therapy licensure in state of Washington.
• Current driver’s license with access to dependable vehicle or public transportation.
• CPR certification preferred or ability to obtain within 90 days of hire.

Responsibilities

• Assess and evaluate patient care needs related to functional status and daily living activities.
• Conduct home assessments and recommend adaptive equipment as needed.
• Collaborate with the health care team to develop and update the therapy plan of care.
• Document CMS-required data and monitor patient progress.
