Job summary

AdvisaCare is hiring a Home Health or Hospice RN in Michigan to provide compassionate, patient-centered care in a one-on-one setting. You’ll perform assessments, implement care plans, and collaborate with families and the care team to improve quality of life.

Qualifications

• Current Michigan RN license in good standing.
• Minimum 1 year home health and hospice experience.
• Empathetic approach to patients and caregivers.

Responsibilities

• Initial and ongoing assessment of patient needs (physical, functional, psychosocial, environmental).
• Implement and revise the individualized plan of care; ensure compliance with policies.
• Educate patients and families on disease processes and self-care; document progress.
• Supervise paraprofessionals; participate in IDG conferences and performance improvement.
• Maintain high customer satisfaction; communicate using current processes and technology.

Requirements

• Initial and on-going assessment of patient's physical, functional, psychosocial and environmental needs
• Implement the individualized plan of care and revisions as needed
• Compliance with AdvisaCare Policies and Procedures, Medicare compliance
• Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concern
• Supervision of paraprofessionals providing services to patient
• Preparing clinical and progress notes, participating in IDG conferences
• Participate in Home Health and Hospice performance improvement program
• Maintain high level of customer satisfaction with our clients, families and caregivers
• Communication of information using current process and technology within organization

Requirements

• Current Michigan License in good standing
• Minimum 1 year home health and hospice experience
• Empathetic approach to patients and caregivers
• Participate in field visits daily, weekly office meetings as indicated
• Ability to adapt to patient's status and needs
