Job summary

Ascend Home Health is seeking an experienced and compassionate Registered Nurse (RN) On-Call to provide skilled nursing support across Essex and Bergen Counties during evening and overnight hours. The role includes responding to patient needs, performing home visits when needed, and coordinating care with physicians and the health care team.

Responsibilities include managing on-call visits, documenting care, educating patients, and ensuring compliance with physician orders and regulations.

Qualifications

• Must have a State Certified RN license.
• Experience in Hospice or home health care, REQUIRED
• Must have excellent documentation skills and the ability to work independently.
• Strong communication skills.
• Valid driver's license & reliable transportation.
• Must be available to travel locally within assigned territory.

Responsibilities

• Assume responsibility for assigned patient visits and the on-call schedule, including the assessment, planning, implementation, and evaluation of nursing care.
• Respond to patient and caregiver calls during assigned on-call hours and determine the appropriate clinical intervention.
• Complete skilled nursing home visits as needed throughout the assigned territory.
• Assess patients' physical and clinical status and identify changes requiring intervention.
• Regularly re-evaluate patients' nursing and home health needs.
• Provide skilled nursing care in accordance with physician orders and the patient's plan of care.
• Communicate changes in patient condition to the physician and care team.
• Coordinate care with physicians, therapists, home health aides, and other professionals.
• Provide patient and caregiver education regarding medications, treatments, disease management, safety, and the plan of care.
• Administer medications and treatments per orders and regulations.
• Maintain accurate clinical documentation at the point of service.
• Maintain regular communication with the direct supervisor.
• Identify patient needs for additional services and resources.
• Participate in on-call responsibilities per policies.
• Maintain patient confidentiality and HIPAA compliance.
• Travel locally throughout Essex and Bergen Counties to provide care.

Ascend Home Health is seeking an experienced and compassionate Registered Nurse (RN) On-Call Nurse to provide skilled nursing support to patients throughout Essex and Bergen Counties during evening and overnight hours.

The RN On-Call Nurse is responsible for responding to patient and caregiver needs during assigned on-call hours, completing home visits when clinically necessary, assessing changes in patient condition, and coordinating care with physicians and other members of the healthcare team.

• Regularly re-evaluate patients' nursing and home health needs.
• Provide skilled nursing care in accordance with physician orders and the patient's individualized plan of care.
• Communicate changes in patient condition to the physician, clinical supervisor, and other appropriate members of the care team.
• Coordinate care with physicians, therapists, home health aides, and other healthcare professionals as appropriate.
• Provide patient and caregiver education regarding medications, treatments, disease management, safety, and the established plan of care.
• Administer medications and treatments in accordance with physician orders, applicable federal and state regulations, and Ascend Home Health policies.
• Perform skilled nursing procedures and treatments within the RN's scope of practice.
• Maintain accurate, timely, and complete clinical documentation at the point of service in accordance with agency policies and applicable Medicare and regulatory requirements.
• Maintain regular communication with the direct supervisor regarding assignments, patient concerns, changes in condition, and other clinical matters.
• Identify patient needs for additional services, medical equipment, supplies, or other resources and coordinate appropriately.
• Participate in on-call responsibilities in accordance with Ascend Home Health policies and procedures.
• Maintain patient confidentiality and comply with HIPAA and all applicable regulatory requirements.
• Travel locally throughout Essex and Bergen Counties to provide patient care as needed during assigned on-call hours.
