Phelps Health in Rolla, MO seeks a part-time Occupational Therapist to develop and implement patient-specific plans of care in home health or hospice settings. You will evaluate patients, set measurable goals, and coordinate with the care team to improve functional outcomes and independence.

The role requires MO licensure, AOTA certification and strong communication skills. Prior hospital and rehab experience is preferred; travel to patient homes is expected as needed.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Missouri OT license and AOTA certification; CPR.
• Minimum one year experience in a JCAHO-accredited hospital; acute rehab experience; home care experience preferred.

Responsabilités

• Evaluates patients for admission to home health or hospice per CMS or payer guidelines.
• Develops an age-appropriate plan of care focused on self-management and measurable outcomes.
• Communicates with patients' physicians about progress and updates plans of care.
• Supervises COTA and educates homecare staff on the plan of care as indicated.
• Conducts timely documentation and ensures adherence to infection control procedures.
