Job summary

Qualifications

• Minimum one year experience in a JCAHO-accredited hospital; acute rehab experience; home care experience preferred.

Responsibilities

• Evaluates patients for admission to home health or hospice per CMS or payer guidelines.
• Develops an age-appropriate plan of care focused on self-management and measurable outcomes.
• Communicates with patients' physicians about progress and updates plans of care.
• Supervises COTA and educates homecare staff on the plan of care as indicated.
• Conducts timely documentation and ensures adherence to infection control procedures.
