Job summary

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Evidence of ongoing education; AOTA membership encouraged.
• Licensed in Indiana to practice OT.
• May supervise/training personnel such as COTA.
• CPR certified or willing to obtain certification.
• Experience in home health or health care setting recommended but not required.

Responsibilities

• Evaluate patient within 24 hours of referral.
• Recommend programs patient-specific to diagnosis and age.
• Plan, administer and supervise the COTA in the treatment plan.
• Communicate with referring physician and rehabilitation team.
• Maintain an accurate daily record of treatments.
• PRN.
