Job summary

Lifespan Therapy is seeking an in-home Home Health Occupational Therapist in California. You will perform in-home ADL assessments, address home safety, and implement adaptive strategies to promote independence. The role offers flexible scheduling and the chance to work with a supportive team to coordinate care.

Required are a valid CA OT license, CPR/BLS, and a driver’s license with auto insurance. TB testing and liability/business licenses may be needed.

• Driver's license and auto insurance are required.
• TB test within the last year may be needed.
• Professional liability and business license (if applicable).

Responsibilities

What You'll Do

• Provide Skilled OT Care: Perform in-home assessments to address activities of daily living (ADLs), instrumental ADLs, and home safety.
• Develop Adaptive Strategies: Create personalized treatment plans focusing on energy conservation, use of adaptive equipment, and functional strengthening for daily tasks.
• Manage Your Visits: Schedule and conduct patient home visits based on your preferred caseload and availability.
• Collaborate for Coordinated Care: Work with a supportive team to ensure a holistic approach to each patient's plan of care.
