Job summary

Staffed Therapy, LLC is seeking highly motivated Occupational Therapists to join our home health team serving Southern California. You will evaluate and treat patients in their homes, develop personalized therapy plans, monitor progress, educate families, and coordinate with nurses and case managers to ensure continuity of care.

We emphasize evidence-based practice, compassionate care, and patient empowerment.

Qualifications

• Extremely organized and detail-oriented.
• Strong communication and collaboration skills.
• Comfortable using online tools and documentation.
• Prior experience in home health or rehabilitation is a plus.

Responsibilities

• Provide occupational therapy services to patients in clients' homes.
• Develop and monitor individualized therapy plans.
• Educate patients and caregivers on activities and home exercise programs.
• Collaborate with case managers, nurses, and therapists to ensure continuity of care.
