Job summary

Ascension At Home is seeking a Home Health Occupational Therapist to provide high-quality rehabilitative OT services. You will initiate planned activities to enhance physical and psychosocial functioning and collaborate with the care team to coordinate patient needs.

The role includes developing OT plans of care, conducting visits, implementing plans, educating staff and families, evaluating outcomes, and ensuring timely documentation while maintaining patient rights and safety.

Qualifications

• Bachelor of Science in Occupational Therapy required.
• At least one year of experience as an Occupational Therapist in acute care, home health, or rehabilitation setting highly preferred.
• Active and unencumbered OT license in state(s) of employment required.

Responsibilities

• Participates in development of occupational therapy plans of care for patients.
• Makes visits to patients for assessment and evaluations and communicates with physician before start of care.
• Implements the plans of care and provides education to team members, patients, and families.
• Evaluates outcomes of care and supervises paraprofessionals as needed.
• Plans for discharge and participates in agency quality improvement programs.
• Ensures growth and profitability through responsible use of resources and educating the community to our services.
• Completes documentation timely in EMR and coordinates notes and reports.
