Job summary

Palms Home Care in Orlando is seeking an experienced Occupational Therapist to deliver in-home health services aimed at improving patient independence and safety within the home setting. We hire thoughtful professionals who value patient-centered care and collaborative teamwork.

Responsibilities include in-home evaluations, developing and implementing individualized treatment plans, assisting with ADLs, training patients on adaptive equipment and home safety modifications, completing OASIS

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Active Florida OT license.
• Home health experience preferred.
• OASIS experience strongly preferred.
• Strong documentation and time management skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct in-home evaluations and functional assessments.
• Develop and implement individualized treatment plans.
• Assist patients with ADLs (bathing, dressing, grooming, toileting).
• Train patients on adaptive equipment and home safety modifications.
• Complete OASIS documentation as required.
• Collaborate with nurses, PTs, and other team members.
