Job summary

BAYADA Home Health Care is seeking an Occupational Therapist to join our Tucson East Visits Team. The OT will provide skilled in-home therapy, develop care plans, and work with a multidisciplinary team to help clients stay at home.

Eligible candidates must be licensed in Arizona, willing to travel within the territory, and possess or complete an accredited OT program. A sign-on bonus of up to $5,000 is offered and training is provided from day one.

Qualifications

• Be licensed as an Occupational Therapist in Arizona
• Must be willing to travel in assigned territory
• Graduated from an occupational therapy curriculum accredited jointly by the AMA and the AOTA, as indicated by transcript or diploma
• 1 year of experience as an Occupational Therapist preferred

Responsibilities

• Perform thorough evaluations of patients’ functional status and ADL/IADL performance.
• Develop individualized intervention plans to achieve favorable outcomes.
• Evaluate home environment for hazards or barriers to more independent living
• Identify equipment needs and environmental adaptations for enhanced outcomes
• Continually assess and revise the occupational therapy care plan with nursing in the multidisciplinary team
• Educate patients and families on rehabilitative care and activities to promote health and independence
• Accurately document observations, interventions, and evaluations in patient records using a state-of-the-art touch pad tablet

BAYADA Home Health Care is currently seeking an Occupational Therapist (OT) to join our Tucson East Visits Team. OT's work with clients in their home, craft treatment plans, and apply innovative techniques to help them reach their goals. You will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to remain at home. How You’ll Make an Impact:

Perform thorough evaluations of patients’ functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance

Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans to achieve favorable outcomes

Evaluate home environment for hazards or barriers to more independent living

Identify equipment needs and environmental adaptations resulting in enhanced outcomes for clients

Continually assess and revise the occupational therapy care plan and participate with nursing in the multidisciplinary care plan

Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety and independent living

Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided, utilizing a state-of-the-art touch pad tablet
