Job summary

Compassus is seeking a Home Health Occupational Therapist to provide patient-centered care in the home setting across Portage and surrounding areas. You will assess, plan, and implement therapy to help patients perform daily activities safely and independently, while collaborating with families and care teams.

In this role, you’ll evaluate home environments, recommend adaptive equipment, and educate patients on strategies to maximize participation and quality of life.

Qualifications

• Bachelor of Science in Occupational Therapy required.
• Current State License as an Occupational Therapist required.
• Valid Driver’s License required.
• Current CPR certification; TB screen and Hepatitis status required.
• Two years’ OT experience in acute care or rehabilitation setting preferred.

Responsibilities

• Provide one-on-one home health OT services to patients in their homes.
• Help patients and families adapt environments and use equipment to improve participation in daily activities.
• Evaluate home safety and recommend modifications and durable medical equipment.
