Job summary

CRS & Home Health Advantage is seeking a Home Health Occupational Therapist to provide in-home therapy across the Chicago land area. Immediate hire for licensed professionals with a focus on patient-centered care and professional documentation.

You will evaluate, treat, and support patients in their homes, documenting progress and coordinating with families and care teams as needed. Experience is a plus but not required, and independence is essential.

Qualifications

• Licensed OT with current credentials.
• Excellent bedside manner and professional attitude.
• Proficient in documentation and clear communication.
• Able to work independently; experience is a plus but not required.

Responsibilities

• Provide home health occupational therapy services in the Chicago area.
• Document assessments, plans, and progress clearly and timely.
• Coordinate care with patients and families and other care team members.
• Manage caseload with independence and clinical judgment.
