Job summary

Addus Family Company seeks a compassionate Occupational Therapist to provide home and community-based therapy services to seniors and individuals with disabilities. You will assess functional abilities, develop individualized treatment plans, and deliver evidence-based interventions to maximize safety, independence, and quality of life in clients’ homes.

You will collaborate with families, caregivers, and an interdisciplinary healthcare team, provide caregiver training, and document outcomes.

Qualifications

• Licensed OT in the state where practice is required.
• Home health experience is preferred.
• Strong collaboration with families and interdisciplinary teams.

Responsibilities

• Assess functional abilities in clients’ homes and community settings.
• Develop individualized treatment plans.
• Deliver evidence-based occupational therapy interventions.
