Job summary

Avera at Home Pierre is seeking a Certified Nurse Practitioner to provide comprehensive medical care across physical, psychological, and social dimensions in home-based settings.

You will assess, diagnose, and treat patients per approved protocols, educate patients and families, and collaborate with physicians to ensure high-quality, compassionate care. This role requires an active state license and adherence to safety and confidentiality standards in a multidisciplinary team.

Qualifications

• Active NP license in the state of practice.
• DEA registration required for prescribing controlled substances.
• RN license in the state of practice or eligible for reciprocity.

Responsibilities

• Assesses, diagnoses, and treats patients per approved protocol and standards of practice.
• Provides medical services and health education to patients and families to increase health motivation.
• Collaborates with a physician for assessment, diagnosis, and treatment of patients.
• Performs diagnostic and therapeutic procedures per protocols and licensing authorities.
• Interprets diagnostic information and consults with the physician to establish a care plan.
• Ensures appropriate referrals for patients are made.
• Maintains patient safety, confidentiality, and accurate records.
