Qualifications

• Supervise and teach LVNs and HHAs as required.
• Document all care and interventions per law and agency policy.
• Notify healthcare professionals of significant changes in patient conditions.
• Plan and organize work assignments with minimal supervision.
• Participate in departmental quality assurance and utilization review activities.

About This Role

The Home Health Nurse II provides skilled nursing care to patients in their homes, ensuring compliance with agency policies and state regulations. This role is ideal for experienced nurses who are dedicated to patient education and advocacy.

• Pay: $94 - $112/hour
• Location: Novato, California
• Facility: San Rafael Hospital
• Schedule: Full-time, Day shift (Mon-Fri, Weekend rotation every third weekend)

Requirements

Education

• High School Diploma/GED

Experience

• Minimum 2 years of applicable RN experience within the last 5 years, including at least 6 months of home health experience.

Licenses & Certifications

• Current Registered Nurse License in California
• California Driver's License
• Basic Life Support certification required
