The role involves conducting assessments, developing care plans, and providing clinical services such as wound care, drain management, catheter changes, and medication administration.

You will work with a multidisciplinary team, educate patients and families, document care, and ensure safe, high-quality treatment.

Qualifications

• Active New Jersey RN license and ability to work in NJ.
• Current CPR certification required.
• BSN preferred; otherwise have nursing degree.

Responsibilities

• Coordinate and provide skilled home health nursing care.
• Assess health needs and update personalized care plans.
• Perform wound care, drain management, catheter changes, and med management.
• Educate patients and families; navigate community resources.
• Document care accurately and communicate with multidisciplinary team.
