Job summary

Haven-Home-Health in Fort Worth, TX seeks a Medical Social Worker to deliver comprehensive home-based care. You will perform psychosocial assessments, counseling, and referrals to meet client and family needs across all ages.

Work with an interdisciplinary team to coordinate admission, plan resources, and ensure adherence to eligibility standards. Requires MSW, current MSW certification, CPR, and excellent English communication. Local travel and flexible scheduling are involved.

Responsibilities

• Assist in admission by evaluating psychosocial, financial, environmental and community needs.

• Accept assignments within education and competence to meet patient needs.
• Provide effective communication to clients, families, team members and other health professionals; participate in case conferences.
• Maintain/expand competency through ongoing education and in-service programs.
