Qualifications

• Master's Degree in Social Work from an accredited program.
• 2 years healthcare experience preferred.
• Licensed Master Social Worker (MSW) in Oregon or Clinical Social Worker (LCSW) eligibility.

Responsibilities

• Provide direct social work services in Home Health/Hospice using standard clinical process.
• Assess, plan, intervene and evaluate patient and family needs.
• Coordinate care with the inter/multi-disciplinary team and community resources.
