Job summary

Millennium Healthcare is seeking a Licensed Vocational Nurse (LVN) for home health, serving patients in the Southern California area. The LVN will perform patient assessments, provide nursing care, and document care according to the plan of care.

Requirements include a CA LVN license, BLS certification, and 1 year of home health and nursing experience. Per diem schedule with visits across multiple days, weekends, and holidays. Pay is per visit and the role involves on‑the‑road work.

Qualifications

• CA LVN License required.
• BLS from AHA/Red Cross required.
• NSO Nursing License Insurance required.
• Visiting LVN for Home Health: 1 year minimum experience required.
• Nursing: 1 year experience required.

Responsibilities

• Provide nurse visits to home health patients on the road.
• Implement plan of care initiated by RN and monitor patient status.
• Document care provided consistently in the chart.
• Coordinate scheduling with patients and care team.
• Educate patients on care during non-visit days.
• Demonstrate knowledge of regulatory requirements.
