Job summary

One Health Home Health and Hospice is seeking a Licensed Vocational Nurse (LVN) to provide skilled nursing visits in patients' homes across Marin County, CA. The role reports to an RN/physician and supports families while adhering to state regulations.

Qualifications

• Current California LVN License in good standing.
• Current BLS Certification.
• Valid California Driver's License and reliable transportation.
• Home Health experience preferred, but not required.

Responsibilities

Position Summary

The Home Health LVN provides skilled nursing care under the direction of a Registered Nurse and physician, ensuring high-quality patient care in accordance with agency policies and state regulations. The LVN works closely with patients, families, and the interdisciplinary team to promote positive health outcomes and patient independence.

Responsibilities

• Perform skilled nursing visits in patients' homes
• Monitor and report changes in patient condition
