Job summary

Reliant at Home is a multi-site Home Health, Hospice, Caregivers, and Rehab company expanding in Texas. We seek an LVN to join our team and support patient care in home settings under RN supervision.

The role focuses on assessing patient needs, executing the care plan, educating patients and families, and documenting promptly to ensure continuity of care for our senior population.

Qualifications

• LVN license required.
• Home Health experience preferred.

Responsibilities

• Under the direction of the RN, assists in identifying the patient’s physical and environmental needs as evidenced by documentation.
• Participates in planning and implementing care in partnership with the patient, family, caregiver, and the RN. Provides services that are ordered by the physician as indicated in the plan of care.
• Provides patient, family, and caregiver education.
• Provides effective communication to the patient, family, team members, and other health care professionals as evidenced by clinical notes, case conferences, and communication notes.
• Completes appropriate documentation within 24-48 hours.
