You will follow the plan of care, uphold infection control standards, and collaborate with the care team to deliver compassionate, high-quality nursing services.

The role requires an active LVN license, a valid driver's license or transportation, and strong communication skills. A $5,000 sign-on bonus is offered for full-time joining the team.

Qualifikationen

• Current and unrestricted LVN licensure in state of practice.
• Current driver's license and vehicle insurance or access to dependable transportation.
• CPR certification preferred or able to obtain within 90 days.

Aufgaben

• Provides high quality clinical services within the scope of practice, as defined by state laws governing nursing, per plan of care, coordinating with health care team.
• Documents patient visits thoroughly in the electronic medical record and claims data entry system.
• Participates in care coordination, reports patient needs and changes in condition, attends case conferences and communicates with patient, family, physician, and care team.
• Function in diverse home settings regardless of age, race, creed, color, sex, disability, or financial condition.
