Job summary

SA Nurses is expanding and hiring Licensed Vocational Nurses for in-home patient care across Kentucky. You will manage a heavy caseload while delivering compassionate, individualized care in patients' homes.

Under supervision of a Registered Nurse and Director of Nursing, you will implement plans of care, document thoroughly, and coordinate services across the care team. Travel to multiple patients per day is expected.

Qualifications

• Active LVN license required
• 1–2 years in-home care experience preferred

SA Nurses is rapidly expanding, and we are hiring LVNs urgently. We work with patients of all ages and backgrounds who require daily care in their homes. We hire the most compassionate and thoughtful talent in the field. You will be responsible for managing a heavy caseload, so time management skills are essential. However, it is important to provide each patient with the time and attention necessary to meet his or her needs. If you enjoy working one-on-one with patients outside of a clinical setting.

Licensed Vocational Nurse Responsibilities:

• Under the supervision of a registered nurse, provides quality care to patients and performs technical skilled care in patient's home in compliance with nursing standards, federal, state, local government regulations, and company clinical policies.
• Provides direct and individualized nursing care to assigned patients based on nursing standards and under the supervision of the Director of Nursing and/or a Registered Nurse designee.
• Ensures quality and safe delivery of home health services to patients/clients and families/caregivers.
• Implements a plan of care formulated by a physician and Registered Nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient/client and family caregiver education and information pertinent to diagnosis and plan of care.
• Maintains a safe and healthy working environment and uses safe work methods and procedures in accordance with company clinical standards.
• Participates in coordination of home health services with Registered Nurse and clinical management.
• Assists the patient in learning appropriate self-care activities
• Uses equipment and supplies effectively and efficiently.
• Documents and reports activity as required.

Qualifications:

• State registered nurse license required
