Qualifications

• Licensed Vocational Nurse (LVN) in the state of Texas (required).
• Minimum of 1 year of home health experience preferred.
• Strong knowledge of patient care, infection control, and documentation practices.
• Ability to follow and implement a Plan of Care under RN supervision.
• Strong clinical assessment, documentation, and communication skills.

Responsibilities

• Provide skilled nursing care to patients in the home setting under the supervision of a Registered Nurse.
• Assist in identifying patient physical, psychosocial, and environmental needs.
• Participate in developing and implementing the Plan of Care in collaboration with the RN.
• Administer treatments and interventions per physician orders and agency policies.
• Monitor patient condition and report changes to the supervising RN.
• Maintain accurate and timely clinical documentation in compliance with regulatory standards.
• Participate in interdisciplinary communication and weekly case conferences (IDG/IDT).
• Provide verbal and written updates to the RN prior to reassessment, supervisory, and discharge visits.
• Follow infection control and safety protocols (OSHA standards).
• Ensure patient safety and quality of care at all times.
• Comply with all state regulations and professional standards of nursing practice.
• Participate in in-service training and maintain clinical competency.
• Meet productivity expectations and ensure documentation is completed within required timeframes.

Qualifications:
