Job summary

All Ways Caring HomeCare seeks an LPN/LPT/LVN to provide direct and indirect nursing care in clients' homes, following physician orders and care plans. You will educate clients, coordinate with the care team, and perform duties under RN supervision to ensure safe, effective home-based health services.

The role requires valid nursing licensure in the operating state, medication administration experience, and strong communication skills to support patients and families in maintaining independence

Qualifications

• Degree in Nursing from an accredited school.
• Experience in administering medications.
• One or more years of nursing experience preferred.
• Home health care experience preferred.
• Current and unrestricted license in the state of the operation.
• CPR professional certification.
• Basic clinical skills.
• Professional appearance and demeanor.
• General knowledge of computers and the ability to learn industry software.
• Excellent oral, written, and interpersonal communication skills.
• Excellent organizational and time management skills with attention to detail.
• Ability to work independently or in a team.
• Ability to develop critical thinking and problem-solving.

Responsibilities

• Provides skilled nursing care per physicians orders and Plan of care.
• Documents nursing actions, health status, and communicates with family and physicians.
• Documents progress and outcomes for established goals and informs physician and families.
• Administers medications and performs medication management as needed.
• Provides health teaching and assists client in learning self-care techniques.
• Conducts nursing assessment visits as needed.
• Follows regulatory standards and operation procedures for incidents.
• Works with RN, Director of Nursing, and Branch Manager on planning and evaluating care.
• Attends staff meetings and trainings.
• Ensures compliance with state/local/payer requirements and proper documentation.
• Transcribes or ensures transcription of physician orders.
• Maintains furnishings, equipment, and supplies to prevent disease spread.
• Schedules and coordinates appointments and responds to findings.
• Communicates with supervisor and scheduler about hospitalized clients.
• Provides new employee training and competency validation.
