Qualifications

• Graduate of an approved Practical Nursing (LPN) program.
• Active LPN license in the applicable state.
• State certifications and continuing education as required.
• Experience in home health, long-term care, or senior living preferred.

Responsibilities

• Provide direct patient care in home health and community settings.
• Serve as liaison between Americare Home Health and community teams.
• Assess patient needs and support individualized care plans.
• Implement nursing standards, policies, and care protocols.
• Monitor patient condition and report changes to the care team.
• Collaborate with physicians, nurses, therapists, and leadership.
• Provide guidance and support to caregivers and nursing staff.
• Ensure accurate documentation and regulatory compliance.
