Job summary

Mercy Health Home Care and Hospice by Compassus invites a licensed Practical/Vocational Nurse to join our home health team. You will administer skilled nursing care to patients in their homes under physician orders and the supervision of the Home Health RN Case Manager.

You will implement care plans, document visits, educate patients and families, and participate in team conferences to ensure high-quality home-based care.

Qualifications

• Certificate of an accredited program of Practical or Vocational Nursing as approved by the state.

Responsibilities

• Implements/follows the nursing care plan for each patient.
• Provides nursing services, treatments, and diagnostic and preventive procedures as assigned.
• Observes signs and symptoms and reports to the physician and RN Case Manager.
• Teaches and counsels the patient and family regarding nursing care needs.
• Maintains accurate and complete records of observation, treatments, and care of patients.
• Prepares clinical and progress notes and submits them within 24 hours of visit.
• Interfaces with patients, families, and staff to ensure customer satisfaction.
