Job summary

VitalCaring Group seeks a Licensed Practical Nurse (LPN/LVN) to provide direct home health care. You will work in patients' homes, under RN supervision, to implement care plans, deliver treatments, and support family education.

You will collaborate with a multidisciplinary team, document in the EMR, and ensure infection control. A valid LPN/LVN license and CPR are required, with 1 year of nursing experience preferred.

Qualifications

• LPN/LVN license in the practicing state is required.
• Current CPR certification is required.
• Minimum of 1 year nursing experience preferred (home health, med-surg, or acute care a plus).
• Graduate of an accredited practical/vocational nursing program.

Responsibilities

• Provide direct patient care under RN supervision.
• Implement and support plans of care developed by the RN.
• Deliver skilled nursing services including treatments, medication support, and wound care.
• Monitor patient condition and report changes to the RN and care team.
• Educate patients and caregivers on self-care and disease management.
• Maintain infection control and proper use of equipment and supplies.
• Document patient care accurately in the EMR.
• Communicate with physicians, nurses, and the care team.
• Participate in team meetings and ongoing training.
