KingRecruitingServices seeks an experienced Licensed Practical Nurse (LPN) for home health visits in Oakdale, MN. The role involves delivering skilled nursing care to patients at home under RN supervision and following physician orders.

You will perform wound care, injections, IV therapy, vital signs, and patient education while maintaining accurate medical records. The ideal candidate has at least 1 year of clinical and 1 year of home health experience, is CPR certified, holds an active LPN

Qualifications

• Minimum of 1 year clinical experience.
• Minimum of 1 year home health experience.
• Ability to provide nursing visits per physician plan and maintain medical records.

Responsabilités

• Deliver direct skilled nursing care in patients' homes under RN supervision.
• Execute treatments including wound care, meds, injections, IV therapy, and vitals.
• Monitor patient condition and report changes to RN or physician.
• Collaborate with RN Case Manager on care plan developments and updates.
• Educate patients and families on disease process, meds, safety and self-care.
• Supervise Home Health Aides and provide feedback on tasks.
• Complete timely clinical documentation per agency and payer guidelines.
• Participate in interdisciplinary case conferences and quality initiatives.
• Ensure safety and infection control protocols during home visits.
• Manage travel and visit scheduling within the territory.
