Job summary

NHC HomeCare Lawrenceburg in Lawrenceburg, TN is hiring a Full-Time & Part-Time LPN to provide skilled nursing services under the supervision of a Registered Nurse, following the plan of care and state regulations.

Qualified candidates hold an unencumbered LPN license, graduate from an LPN program, with at least 1 year of clinical experience; those with less will be mentored through a 1-year period. Home health care experience is desirable.

Qualifications

• Unencumbered LPN license in the state where the agency operates or compact state
• Graduate of a state approved LPN program
• Minimum of one year clinical experience as an LPN
• Individuals with less than one year of experience will be mentored through the end of that year
• Experience in home health care is desirable

Responsibilities

• Under RN supervision, provides skilled nursing services per plan of care and state acts
• Instructs patient/caregiver in care and disease management; documents comprehension and compliance
• Observes patient responses and revises the plan with RN input
• Performs duties per plan of care and policy; assists RN/Clinical Manager as assigned
