Job summary

Premier Home Health Providers Inc. seeks a per diem Licensed Practical/Vocational Nurse (LPN/LVN) in California to provide direct patient care in home health settings. You will follow individualized care plans, educate families, and document care for timely handoffs.

Travel to patient homes is required with a compassionate, patient-first approach. Minimum 3 years of nursing experience, California LPN/LVN license, CPR (AHA), and a valid driver’s license are essential.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license and reliable insured vehicle.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
