Qualifications

• 1 year of clinical LPN experience required.
• Current LPN license for assigned state(s).
• Current CPR certification required.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to meet patient needs and notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs during visits.
• Review medications and perform medication reconciliation.
• Perform physician-ordered procedures (e.g., Foley catheter change).
• Prepare equipment and materials using aseptic technique and document timely.
