Responsibilities

Provides high quality clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care and in coordination with the health care team.
Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record.
Actively participates in the agency's care coordination process including timely reporting of patient needs and changes in condition.
Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
